Whiplash - are doctors giving you the right treatment?
Whiplash is a condition that can be sustained surprisingly easily, usually after a rear impact collision. Because it involves injury to the muscle tissues surrounding the vertebrae, the conventional methods of treatment can involve the prescription of analgesics and the wearing of a soft neck brace. However, a doctor from the University of South Australia’s (UniSA) School of Health Sciences has made a number of discoveries that look set to challenge that convention.
What happens during an impact
During a rear impact collision the head is thrown forward as the body absorbs the momentum of the impact. It stretches the spine beyond its usual range of motion before the head is then thrown back in the opposite direction. In this action the spine mimics the motion of whip, giving the injury its name. Damage primarily occurs to the soft tissues that surround the spinal column. Muscles are stretched, bruised or torn and the ligaments connecting the discs and vertebrae are often sprained. Whiplash symptoms include headaches, dizziness, blurred vision and pain in the neck and back. Often these symptoms become manifest within hours of an impact, but they can take weeks or even months to make themselves known.
A soft collar - the traditional treatment
Whiplash is often defined as a ‘self-limiting’ condition, meaning that it can get better with minimal treatment. However, in cases where treatment is deemed to be a priority, many clinicians opt to use a soft collar to restrict movement of the neck in the belief that it is better to allow the muscles to rest and repair themselves. Despite this, many patients remain in constant pain and can go to develop further complications such as degeneration of the spinal points.
Dr. Saravana Kumar of the UniSA believes that the use of soft collars does more long-term harm than good. He says that: “the use of soft collars for treatment of whiplash was originally developed when it was believed that whiplash could be managed like a broken bone that is immobilising the spine. However, new research evidence has disproved this theory but the use of soft collar in clinical practice continues to persist. Evidence shows that wearing soft collars is actually harmful and it is staying active which promotes recovery in whiplash.”
Challenging accepted practice
Dr. Kumar’s findings are the result of two years’ worth of research involving 34 physiotherapists and 16 chiropractors and funded by National Health and Medical Research Council (NHMRC) - National Institute of Clinical Studies (NICS) - Motor Accident Commission (MAC) Fellowship. His research has also unearthed a number of other practices that he believes to be unhelpful. “Apart from the soft collar treatment for whiplash, having an x-ray for lower back pain and getting an antibiotic prescription for the common flu are other frequently requested treatments that should not be used.”
These findings look set to challenge many of the set paths that clinicians and medical professionals have relied on for years. However, it seems that the UK has been at the forefront of this silent revolution. Physiotherapy has been a recommended treatment for whiplash for some time. Anyone who suffers a whiplash injury should feel that they have the right to question the treatment they are being given.
